Action Awards Cycle 4 Final Report Presentations - Round 2

Between January 2016-December 2016, nine interprofessional teams implemented evidenced-based practice change projects focused on improving the health of older adults and their families. Each of these project teams was a recipient of a one-year Action Award grant of up to $10,000 as part of the John A. Hartford Foundation’s Change AGEnts Initiative. On January 12, four of these teams will present their final reports, highlighting their results, successes and lessons learned. On January 19, the five other teams included in this fourth cycle of funding will present their reports. Following each brief presentation, a panel of healthcare practice and policy thought leaders will engage in a Q&A with the project team and provide feedback. Audience members will also have the opportunity to pose questions to the project teams.

About the Action Awards

The John A. Hartford Foundation Change AGEnts Action Awards were one-year grants for up to $10,000 that were available to interprofessional teams led by John A. Hartford Foundation alumni for the purpose of achieving meaningful change to practice or policy that will improve the health and wellbeing of older adults and/or their families. For this Initiative, we viewed changing practice as a collaborative and interprofessional process that uses strong evidence to achieve results. Between August 2014 and December 2016, we funded 34 projects over the course of four cycles. Read summaries of all the projects here.
