CARE Act Toolkit and Guides to Assist NY Hospitals and Patients

United Hospital Fund has developed a hospital staff toolkit and two patient and family caregiver guides to help hospitals comply with New York State’s Caregiver Advise, Record, and Enable (CARE) Act. This legislation was developed by AARP and enacted in April 2016.

Implementing New York State’s CARE Act: A Toolkit for Hospital Staff is designed to support hospital staff as they integrate the law into their daily routine. It addresses the four key requirements of the CARE Act, related to identification, engagement, and preparation of family caregivers, as well as follow-up care after discharge.

New York State’s CARE Act: A Guide for Patients and Caregivers explains what patients and families can expect to be asked by hospital staff and what instructions they are entitled to receive in advance of the transition from hospital to home. In both full-length and abridged versions, the guides are available in English, Spanish, Chinese, and Russian.
