The New England Journal of Medicine recently published a perspective piece on health care models that deliver care in a patient’s home rather than the hospital.
Over nearly two decades, The John A. Hartford Foundation supported the Hospital at Home model, from its development at Johns Hopkins University, through multi-site testing, to early dissemination.
