Prepare For Your Care New Website

Prepare for Your Care grew out of a desire to help make medical decision making easier for people and caregivers.

Information on the new Prepare website includes:

  • A video introducing why and how people can use PREPARE;
  • The PREPARE pamphlet, which provides an easy way to introduce patients to advance care planning;
  • The PREPARE Toolkit, which helps community groups set up and run a PREPARE session; and
  • Access to the easy-to-read Advance Directive for California.
