Nurses Improving Care for Healthsystem Elders in Long-Term Care (NICHE-LTC)

Grant Summary

This grant aims to improve the care of older adults in nursing home, assisted living and post-acute rehabilitation settings through the expansion and adaptation of the successful and self-sustaining NICHE hospital program, a model developed with support from the John A. Hartford Foundation. This grant project will focus on the development of comprehensive curricula, assessment tools, mentoring resources, and a quality designation process to support organizational change and improvement in the care of older adults. Over three years, 225 nursing homes will join NICHE-LTC, participate in geriatric quality improvement activities, and build staff competence in care for older adults, while building sufficient revenue for ongoing sustainability.
