Invitation to Join: Age-Friendly Health Systems Action Community

The Institute for Healthcare Improvement (IHI) is inviting health systems to join the next Age-Friendly Health Systems Action Community that starts in March 2022. Registration will be open until April.

Becoming an Age-Friendly Health System entails reliably providing a set of four evidence-based elements of high-quality care, known as the “4Ms” — What Matters, Medication, Mentation, and Mobility, to all older adults in your system. The 4Ms make care of older adults that can be complex, more manageable.

An Action Community brings teams from across different health systems together to accelerate their own description and adoption of the 4Ms. Guided by expert faculty and an “all teach all learn model,” teams participate on monthly webinars, attend a virtual convening, and test specific changes to improve care for older adults. The process is designed as an on-ramp for hospital-based teams (e.g., emergency departments, intensive care units, general wards, medical-surgical units) and ambulatory care teams (e.g., primary care, specialty care) to test and adopt age-friendly care.

During the 7-month virtual learning community, a team will test the 4Ms framework at their site of care (e.g., a hospital unit, primary care practice, nursing home) with the goal of being recognized as an Age-Friendly Health System – Committed to Care Excellence. New and previously engaged health systems are welcome to join. Previously engaged health systems often join another Action Community to support their efforts to spread age-friendly care to additional care locations in their system.

Age-Friendly Health Systems is an initiative of JAHF and IHI in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).
