AAHCM Video: The Effects of Care - Hear it from the Patients and Providers Benefiting from HBPC
The American Academy of Home Care Medicine (AAHCM) has released a video featuring people who have benefitted from home-based primary care (HBPC) solutions.
AAHCM's 'The Effects of HBPC' is a multifaceted awareness and education campaign to help payers, providers, and the public better understand the value and cost effectiveness of home-based programs. With HBPC, patients who wouldn't otherwise receive primary care are benefitting from receiving care within their own homes. And providers, by going to their patients' homes, are better able to understand their patients' needs, deliver optimum care that avoids unecessary ambulatory care and emergency department visits, and allow their patients to age at home.
