Administration for Community Living (ACL) Updates
The profiles for ACL’s 2017 Chronic Disease Self-Management Education (CDSME) grantees are now available on the ACL website.
These eight organizations have proposed to collectively reach 38,000 older adults and adults with disabilities through evidence-based self-management programs, empowering them to better manage their chronic conditions.
The organizations that have been awarded the 2017 Prevention and Public Health Fund CDSME grants are:
- AgeOptions (Illinois)
- Big Sandy Health Care (Kentucky)
- Elder Services of the Merrimack Valley (Massachusetts)
- Health Foundation of South Florida
- Partners in Care Foundation (California)
- Rhode Island Department of Health
- Utah Department of Health
- Wisconsin Institute for Healthy Aging
These organizations include John A. Hartford Foundation grantees, Elder Services of the Merrimack Valley and Partners in Care Foundation.
