Apply for Learning Collaborative for Home-Based Medical Care

The American Academy of Home Care Medicine (AAHCM) has released a Request for Application (RFA) to join the National Home-Based Medical Care Quality Improvement Learning Collaborative, led by Drs. Christine Ritchie and Bruce Leff.

5 practices from across the country will be selected to join a year-long participatory quality improvement learning experience. The Learning Collaborative will:

  • Provide transformational strategies and coaching support to help home-based primary care and palliative practices implement quality improvement principles and demonstrate high-value care.
  • Give practices the opportunity to use a CMS-approved Qualified Clinical Data Registry, which employs quality indicators appropriate to the home-based medical care population and care setting.
  • Make available to physicians a quality improvement activity that provides an opportunity for American Board of Internal Medicine-certified physicians to earn 20 maintenance of certification points.
