C-TAC Releases Serious Illness Design and Implementation Framework
The Coalition to Transform Advanced Care (C-TAC), a JAHF grantee, has published a report on the Serious Illness Program Design and Implementation Framework (SIP Framework). Built on available evidence and extant best practices, the Framework offers a flexible tool for care providers to inform the development, implementation, and evaluation of serious illness care programs.
The SIP Framework is designed to:
- Inform serious illness program development, replication, and scaling
- Integrate with care model payment design
- Inform care model and proforma simulator development
- Inform other aspects of design and development such as policy, standardized measurements, and regulatory analysis
