HCCI Launches Nationwide Network for Home-Based Primary Care Education

JAHF grantee, The Home Centered Care Institute (HCCI), has launched Centers of Excellence (COEs) for Home-Based Primary Care with eight leading academic centers across the United States to leverage the first and only comprehensive curriculum in country to help meet the growing need for home-based primary care professionals.

The COE's curriculum is designed for both clinicians and practice managers, consists of classroom training, mentorship, hands-on fellowship and online learning opportunities. The training path is specifically designed to meet the challenges and opportunities encountered in home-based primary care, including 12 hours classroom training, a three month mentorship, and 2-day “Mini-Fellowship,” where clinician learners shadow home-based primary care practitioners on patient visits and practice managers receive additional insight on staffing, scheduling, billing and coding.
