The National Center's Blueprint for Complex Care Survey
The National Center for Complex Health and Social Needs is looking for stakeholders' insight and perspective to help create the Blueprint to determine priorities and key areas of focus for the complex care field.
This second, brief survey is to rank what you believe should be the priority areas of focus for the field over the next 3-5 years. It follows an earlier request for feedback when the National Center gained input on needs, challenges, opportunities and success factors for complex care.
The survey closes May 25.
The Blueprint for Complex Care is a joint project of the National Center for Complex Health and Social Needs, the Institute for Healthcare Improvement (IHI), and the Center for Health Care Strategies (CHCS). The final Blueprint will be shared in the fall of 2018.
