The Institute for Healthcare Improvement (IHI) has released an Age-Friendly Health Systems initiative toolkit for clinicians, "'What Matters' to Older Adults? A Toolkit for Health Systems to Design Better Care with Older Adults."

The "What Matters" toolkit helps celebrate What Matters to You? day. The toolkit is intended to serve as a resource for multidisciplinary care teams, including physicians, nurses, physician assistants, medical assistants, social workers, chaplains, nurse navigators, community health workers, and trained volunteers.

It provides actionable steps and guidance to ensure that every older adult’s health outcome goals and care preferences are understood, documented, and integrated into their care by the entire health care team. Supported by The SCAN Foundation, the toolkit was created as part of the Age-Friendly Health Systems initiative.
