Webinar: Building Sustainable CBO and Health System Partnerships under Medicaid Delivery System Refo

The National Association of Area Agencies on Aging (n4a)-led Aging and Disability Business Institute and the American Society on Aging (ASA) are holding a webinar on October 23, "Building Sustainable CBO and Health System Partnerships under Medicaid Delivery System Reform."

A partnership, Care Transition Program, between NYC Department of Aging’s contracted community-based organizations and two of OneCity Health’s hospitals has been successful in achieving financial recognition for infusing a socially oriented service into a healthcare setting. The webinar will discuss the collaboration and lessons learned, as well as the possibility of future collaborations once statewide reform efforts have been completed.

Speakers include:

  • Ishmael Carter, senior assistant vice president and hub executive director, OneCity Health network of partners in Queens and Manhattan
  • Carin Tinney, director of Program Development, NYC Department for the Aging.
  • Meghan Shineman, MPA, healthcare planning specialist, NYC Department for the Aging

NYC DFTA received the 2018 Honorable Mention for The John A. Hartford Foundation 2018 Business Innovation Award.
