Grantmakers in Health (GIH) and Grantmakers in Aging (GIA) are holding a webinar, "Blueprint for Complex Care: Opportunities for Philanthropy in Healthcare and Social Services," on April 2.

The webinar will focus on recommendations for strengthening the field to provide quality care for individuals with complex health and social needs, along with how foundations are using the recently released Blueprint for Complex Care in their current and future grantmaking, and how funders can collaborate to improve the lives of these individuals.

Individuals with complex health and social needs often receive care from both medical and social service providers that are expensive, inefficient, and poorly coordinated. The John A. Hartford Foundation (JAHF) is partnering with five other foundations to work together to increase the adoption of evidence-based interventions that will improve the quality of care and lower costs for these individuals.
