Yale News has published an article, "Yale Approach Bases Decision-making on Older Adults’ Own Health Priorities."

The piece talks about results from a pilot project led by teams from three organizations that show that "an innovative approach to health care for older adults with complex health needs can be integrated into a real-world clinical practice. Known as patient priorities care, the approach helps patients and clinicians focus decision-making and health care on what matters most - namely patients’ own health priorities." The approach the team developed is described, and the article links to two recent papers from the pilot project published in The Journal of the American Geriatrics Society.

The John A. Hartford Foundation co-funds implementation and testing of the Patient Priorities Care model.
