An article from Catholic Health Association (CHA) features the Age-Friendly Health Systems initiative.

The article discusses the Age-Friendly Health Systems initiative's fundamental "4Ms": What Matters, Mobility, Medications, and Mentation. The first phase of this initiative is to test prototypes among five major health systems - Trinity Health, Ascension, Providence St. Joseph Health, Anne Arundel Health System, and Kaiser Permanente.

"Our goal is to close the gap between what we know and what we do when it comes to our older patients," said Ann Hendrich, senior vice president and chief quality/safety and nursing officer for St. Louis-based Ascension. "We want to do the right thing for each patient, each time and we need to quantify this."
