The blog details the initial stages of the Age-Friendly Health Systems model, and the rapid movement into the next stages of the initiative via an Action Community of health care teams.

Dr. Fulmer makes the point that community-based organizations have long-standing & deep expertise in age-friendly care and play a pivotal role, and invites community-based organizations to join the Age-Friendly Health Systems Action Community as vital partners.
