Medical Home News: Catching Up With Marcus Escobedo

Marcus Escobedo, MPA, Senior Program Officer and Communications Director at The John A. Hartford Foundation, spoke with Medical Home News Editor, Ron Shinkman, about the roles medical homes play in geriatric and other forms of care.

"RS: What kind of roles do medical homes play in providing care to senior populations?

ME: I think there are a couple of areas where we really see better outcomes among older adults in models like the medical home. Much of that has to deal with appropriate and effective care coordination. With an older adult, we typically end up with multiple chronic conditions and they have to see different specialists. Having a strong primary care base in a patient-centered medical home can help older adults in particular manage the different treatments, medications and approaches being used by all these various specialists.

That in turn can lead to reductions in unwanted care, and reductions in unnecessary prescriptions."
