Reducing Avoidable Hospital Readmissions

A new report, “Health Care Leader Action Guide to Reduce Avoidable Readmissions,” co-funded by the John A. Hartford Foundation and the Commonwealth Fund, and co-authored by the Health Research and Educational Trust of the American Hospital Association, offers an overview of the top research and practice models that have succeeded in reducing avoidable hospital readmissions. This information will help hospital leadership adapt these strategies to their own needs, and not only hold down rising health care costs, but provide better quality care to hospital patients as well – the vast majority of whom are older adults.
