Terry Fulmer, PhD, RN, FAAN, President of The John A. Hartford Foundation, has written a blog for Next Avenue, "What Matters? Don’t Let Health Care Get in the Way."

Start with "asking yourself what matters most to you, discussing that with family and others and demanding that your health care providers respond accordingly," Dr. Fulmer says. She discusses models of health care that put individual care goals at the center, such as Age-Friendly Health Systems, community-based palliative care, and Programs of All-Inclusive Care for the Elderly (PACE).

"An age-friendly health system would prioritize asking and acting upon what matters to us, especially as we age and become seriously ill."
