Age-Friendly Health Systems Initiative: Implementation Guide to EHR Requirements for Adoption of the 4Ms

The Age-Friendly Health Systems initiative has released an implementation guide, "Age-Friendly Health Systems: Guide to Electronic Health Record (EHR) Requirements for Adoption of the 4Ms."

The implementation guide was designed as a resource for health systems to build the Age-Friendly Health Systems (AFHS) framework, and associated care practices, into the EHR. This framework, known as the 4Ms, is what matters, medication, mentation, and mobility.

The guide addresses how to incorporate both the “assess” and “act on” drivers of the 4Ms into the EHR. The guidance is not specific to a particular EHR vendor, there are 2 versions, one with specific suggestions and examples that apply to the Cerner platform, and a second version applicable to the EPIC platform.
