Invitation to Join: HANYS 2023 NY Age-Friendly Health Systems Action Community

The Healthcare Association of New York State (HANYS) is inviting health systems teams to join the 2023 New York Age-Friendly Health Systems Action Community.

The Action Community helps participant health systems accelerate the adoption of the Age-Friendly Health Systems 4Ms Framework (What Matters, Medication, Mentation and Mobility) care, using an “all teach, all learn” model. Participant teams receive group education through monthly webinars and topical coaching calls and attend a half-day virtual summit.

The benefits of joining the Action Community include:

  • Redeploying and prioritizing existing health system resources to deliver high-quality, evidence-based care to older adults;
  • Becoming recognized as an “Age-Friendly Health System;” and
  • Improved patient and clinician experience and more efficient, lower-cost care.

Thanks to the generous support of the Fan Fox & Leslie R. Samuels Foundation, New York State Health Foundation, New York Community Trust, and Health Foundation for Western & Central New York, there is no fee to participate in the New York State Action Community. Additionally, Age-Friendly Health Systems participants may apply for a $10,000 stipend to support this work (up to 20 teams will be awarded stipends). Scholarships are also available to geriatric EDs new to accreditation (up to 18 hospitals will receive funding on a first-come, first-serve basis).

The Action Community will start in January 2023.
