Terry Fulmer, PhD, RN, FAAN, President of The John A. Hartford Foundation, has written an article for Next Avenue, "Age-Friendly Health Care: Speaking Up About What Matters to You."

As part of the JAHF-sponsored series on age-friendly health care, Dr. Fulmer writes about how as we get older, we need to take an active role in our health care, telling our doctors and nurses what matters most to us and what health goals we want to achieve.

She discusses how the Age-Friendly Health Systems initiative 4Ms framework give us a way of talking to our health care providers, who must respond differently.

"The outdated idea of thinking about 'what’s the matter with the patient?' must give way to 'what matters to the patient' as people live longer than ever. This is a goal of the Age-Friendly Health Systems initiative," writes Dr. Fulmer.
