We were very pleased to see that America’s Healthy Futures Act, the health care reform bill released by the Senate Finance Committee on September 16, 2009, included a provision for a pilot program called the Community Care Transitions program, intended to reduce re-hospitalizations of high-risk patients. The language is based on the Care Transitions Program developed by Dr. Eric Coleman, professor of medicine at the University of Colorado, with funding from the Hartford Foundation. We are pleased that lawmakers have recognized the significance of Dr. Coleman’s work, and the effectiveness of the Care Transitions program, which helps smooth the transition from hospital to home for high-risk older patients. We hope this terrific program that benefits older adults and saves health care dollars will become part of U.S. public health policy.
