CLIIR Call for Participation: Advancing Age-Friendly Health System Measurement & Evaluation

The Center for Clinical Informatics and Improvement Research (CLIIR) at the University of California San Francisco (UCSF) is providing funding for up to five health systems to participate in a two-year collaborative project to implement the Age-Friendly Health Systems initiative's 4Ms evaluation tools in hospital and ambulatory settings.

Over the past two years, the UCSF team developed and implemented quantitative and qualitative approaches to evaluate 4Ms implementation and impact. During this next phase, up to Age-Friendly Health System sites will be selected to evaluate their 4Ms implementation and impact on outcomes, with support from the UCSF team that has developed a set of evaluation tools.

Specifically, each institution must implement (or expand current implementation of) EHR-based measures of 4Ms adherence in at least one setting. The overall goal is to create scalable evaluation tools that can be used to generate robust evidence on the 4Ms. Each selected institution will receive up to $125,000 in support for the project (July 2024 - June 2026).
