2025 CMS Proposed Age-Friendly Hospital Measure
The Centers for Medicare & Medicaid Services (CMS) released the proposed rule for the fiscal year 2025 Medicare hospital inpatient prospective payment system (IPPS) and long-term care hospital prospective payment system (LTCH PPS), which includes the introduction of the Age-Friendly Hospital measure (CMS-1808-P).
The Age-Friendly Hospital measure seeks to ensure that hospitals reliably implement the "4Ms" Framework (what Matters, Medication, Mentation and Mobility) and provide evidence-based elements of high-quality care for all older adults.
This measure evaluates hospitals' dedication to enhancing care for patients aged 65 and above across various hospital settings, such as the hospital ward, operating room, or emergency department. The Age-Friendly Hospital measure includes the following five domains:
- Eliciting Patient Healthcare Goals: This domain focuses on obtaining patients' health-related goals and treatment preferences, which will inform shared decision-making and goal-concordant care.
- Responsible Medication Management: This domain aims to optimize medication management by monitoring the pharmacological record for drugs that may be considered inappropriate in older adults due to increased risk of harm.
- Frailty Screening and Intervention: This domain aims to screen patients for geriatric issues related to frailty, including cognitive impairment/delirium, physical function/mobility, and malnutrition, for the purpose of early detection and intervention where appropriate.
- Social Vulnerability: This domain seeks to ensure that hospitals recognize the importance of social vulnerability screening of older adults and have systems in place to ensure that social issues are identified and addressed as part of the care plan.
- Age-Friendly Care Leadership: This domain seeks to ensure consistent quality of care for older adults through the identification of an age-friendly champion and/or interprofessional committee tasked with ensuring compliance with all components of this measure.
Comments were invited by June 10, (reference file code CMS-1808-P), and organizations were invited to sign on to JAHF's letter of support, affirming their organization’s support. Organizations that have expressed support include AARP, AGS, CAPC, EDC/NDCC, GSA, IHI, NAC and West Health Institute among others.
