JAHF New Partners Program Call for Proposals:
National Initiatives Advancing Improvements in Care for Older Adults
Overview
The John A. Hartford Foundation (JAHF) is entering a new chapter that builds on its work to improve care for older adults and support family caregivers while bringing new organizations, leaders and ideas into its community of grantees and partners.
Through this funding opportunity, JAHF is seeking proposals from organizations that have not previously received a major grant from the foundation.

What types of initiatives is the foundation seeking?
JAHF welcomes proposals for national or multi-site initiatives addressing a wide range of important and timely issues that impact older adults' care and/or family caregivers.
Proposals must align with JAHF’s three priority areas but are not limited to specific topics or settings.
Successful proposals will include evidence-informed models of care or policy implementation activities that are scalable, sustainable, and clearly aligned with one or more of the foundation's priority areas.
How much funding is available?
Up to 10 grants will be awarded, each providing up to $300,000 over two years ($150,000 per year).
Who is eligible to apply?
Applicants must be organizations that have not previously received a major grant from JAHF (see our FAQ for additional details). Organizations that have received one-time discretionary awards remain eligible.
Eligible applicants must also:
- Be U.S.-based 501(c)(3) public charities, colleges or universities, hospitals, or health care organizations conducting work in the United States
- Propose work that primarily benefits older adults and/or family caregivers in the United States and has a national or multi-site scope
- Comply with applicable federal restrictions on lobbying
- Submit only one proposal
What will not be funded?
This funding opportunity will not support:
- Single-site projects
- General operating support or capital projects (construction or renovation)
- Programs whose principal purpose is the delivery of direct services, such as routine clinical care, legal representation, transportation or meals
- Films or documentaries, books, or art projects
What is the application process?
Grants will be awarded on a competitive basis. The proposal process will have two stages: letter of intent and full proposal.
The number of applicants invited to submit full proposals will depend on the strength of the applicant pool. An invitation to submit a full proposal does not guarantee funding.

Letter of intent deadline:

Notifications of full proposal invitations:
By Jan. 15, 2027, at noon ET
Full proposal deadline:
March 3, 2027, at noon ET
Awards announced:
By June 11, 2027

July 1, 2027
