The National Center for Equitable Care for Elders (NCECE) held a webinar on April 15, "Health Centers as Age-Friendly Health Systems."

The webinar focused on how health centers can use the Age-Friendly Health Systems initiative '4Ms' framework (What Matters, medication, mentation, mobility) to build on the tremendous care health centers already provide to older adults to make sure evidence-based care, grounded in What Matters to older adults, is practiced reliably with every older adult, every day.

Speakers included Leslie Pelton, MPA, Senior Director, Institute for Healthcare Improvement (IHI). Learning objectives were:

  • Describe the importance of aligning care with an older adult’s preferences and health outcome goals
  • Know how to identify your health centers strengths in providing evidence-based care of older adults and champions of that care
  • Identify strategies for reliably incorporating the 4Ms framework into a primary care setting
