The National Center for Complex Health and Social Needs is hosting their annual 'Putting Care at the Center' conference, November 13 - 15, in Memphis, TN.

'Putting Care at the Center' is an opportunity for innovators and advocates for healthcare delivery reform from across the country, both pioneers and newcomers, to meet, network, and create a shared agenda for the emerging field of complex care.

The John A. Hartford Foundation and the American Hospital Association will co-sponsor an Age-Friendly Health Systems session at the conference.

The early bird registration deadline is August 31.
