The National Home-Based Primary Care Learning Network: Wave 7 Request for Applications

The National Home-Based Primary Care (HBPC) Learning Network has announced their Wave 7 Request for Applications, due September 29.

The National HBPC Learning Network seeks to improve medical practice in the home and expand the evidence base for home-based medical care by fostering communities of best practice engaging in quality improvement and conducting applied research.

The Learning Network is looking to add fifteen HBPC practices from around the country to join the sixty-one current Learning Network practices. Eligibility for participation requires engagement in ongoing quality improvement, including attendance at monthly workshops, monthly reporting of quality improvement activities, completion of brief surveys, and active involvement in the Learning Network practice community.
