JAGS Paper What Matters Most April 2025

The Journal of the American Geriatrics Society (JAGS) has published a paper, "What Matters Most: Case Study of Successful Patient-Directed Gabapentin Taper to Improve Mentation, Mobility, and Medications."

The paper is a clinical report of complex geriatric care that shares an example of Patient Priorities Care (PPC) in action. PPC is an evidence-based approach that identifies health priorities and preferences based on patient values. It guides clinicians, patients and care partners in practicing age-friendly health care based on those health priorities. PCC aligns with the Age-Friendly Health Systems (AFHS) initiative and 4Ms framework (what matters, medication, mentation and mobility).

The case study demonstrates how a geriatric provider used the PPC approach to identify what mattered most to a patient and make changes accordingly. The approach helped develop the relationship and trust necessary for patient engagement, creating a “new normal” that better aligned with the patient's goals.
