The Patient Priorities Care project, formerly known as Carealign, has published a paper in the Journal of Aging and Health, "Perspectives of Patients, Clinicians, and Health System Leaders on Changes Needed to Improve the Health Care and Outcomes of Older Adults With Multiple Chronic Conditions."

The paper presents the views of stakeholders including patients, clinicians, health systems, and payers on inappropriate care for older adults with multiple chronic conditions. The stakeholder recommendations formed the basis of the Patient Priorities Care model currently being tested in a large primary care group in Connecticut.
