Yale University Patient Priorities Care (formerly known as Carealign Implementation)

Grant Summary

This renewal proposal represents the second phase of an effort to implement and test the feasibility and sustainability of a prototype for “patient goals-directed” care, a model initially referred to as Carealign. The model was developed with a broad group of stakeholders organized by geriatrics leaders Mary Tinetti of Yale University and Caroline Blaum of New York University under an 18-month planning grant from The John A. Hartford Foundation, in partnership with the Patient-Centered Outcomes Research Institute (PCORI). By aligning care with the patient’s health goals and care preferences, this approach supports high-quality, patient-centered care of complex elders across primary and specialty care. There is no greater value than care and outcomes concordant with a person’s goals and values. In this prototype phase, the model will be implemented and tested within ProHealth, a large primary care group serving the state of Connecticut. The three objectives for the prototyping phase are: 1) implementing patient goals-directed care in practice, 2) producing preliminary evidence for this model of care, and 3) building awareness and demand through stakeholder engagement. The model is now known as Patient Priorities Care.
