Age-Friendly Health Systems

Older adults have unique needs. To prevent harm to older adults, improve health outcomes, and lower overall costs, health care systems must adopt evidence-based models and practices that deliver better care to our rapidly aging population across all settings, including the home and community. 

In partnership with expert innovators and health care leaders, The John A. Hartford Foundation is working to create health systems that are age-friendly and better able to meet the goals of the Triple Aim.

Our grantmaking in this area will:

  • Develop large-scale approaches that help health systems transform care;
     
  • Disseminate evidence-based models of care and best practices; and
     
  • Connect hospitals and clinics to community-based services and supports.
