Age-Friendly Health Systems Case Studies: Home Health Care

The Institute for Healthcare Improvement has published two new case studies with findings from home health care providers who participated in the Age-Friendly Health Systems (AFHS) home health prototyping initiative.

These resources explore the delivery of age-friendly care based on the 4Ms Framework (What Matters, Medication, Mentation, Mobility) to older adult patients in home health settings.

"How Education Supports Age-Friendly Health Systems in Home Health: Sutter Care at Home" shares learnings from a pilot at one of the largest home care providers in California. As a result of interprofessional collaborative education, the number of older adults receiving 4Ms care at the pilot site from increased 10% to 60% in three months. The case study reviews lessons learned during implementation and describes how using What Matters to guide care planning helped older adults feel heard, while increasing meaning and purpose for care team members.

"The Impact of the 4Ms in Home Health: BAYADA Home Health" shares patient stories and reflections illustrating how the 4Ms Framework can support patient-centered care. The case story also highlights how the organization integrated lessons learned by incorporating the 4Ms in huddles with the care team, where they identified and focused on each patient's goals and values.
