Age-Friendly Health Systems Hospice Resources
The Institute for Healthcare Improvement (IHI) has released new resources to help hospice organizations implement age-friendly care using the 4Ms Framework (What Matters, Medication, Mentation, and Mobility).
The 4Ms Care Description Worksheet for Hospice Care was created for hospice organizations seeking recognition as an Age-Friendly Health System. The worksheet guides organizations through documenting each of the 4Ms in their current practices and identifying opportunities to strengthen age-friendly care.
The accompanying supplement, Providing Age-Friendly Care to Older Adults in Hospice, provides setting-specific guidance on applying the 4Ms in hospice care, explaining how and why this approach differs from age-friendly home health care and offering practical implementation strategies with advice from the field.
Together, the resources support hospice organizations in delivering person-centered, evidence-based care aligned with what matters most to older adults.
