Leslie J. Pelton, MPA, is a senior program officer at The John A. Hartford Foundation. She is an outcome-oriented leader who catalyzes innovation in health systems and transforms passion into impactful action. She brings more than twenty years of experience partnering with health care leaders to transform how care is delivered and has brought these talents to the improvement of health and health care of older adults and their family caregivers in community, health care practices, hospitals and nursing home-based care. Leslie is a nationally recognized leader in the Age-Friendly Health Systems movement improving the health and health care of older adults across the US.
Prior to joining the Foundation, Leslie was vice president at the Institute for Healthcare Improvement (IHI). At IHI, she collaborated with funders, national partners and leading health systems to design the Age-Friendly Health Systems movement, which has resulted in improved care of older adults in every state across the US. Leslie joined IHI from her own firm, which worked with innovative health care leaders to build new models of integrating research, clinical care and policy driving in improved health outcomes. Leslie also built the Human Capital practice at Deloitte Consulting addressing the human factors associated with adopting strategic and operational innovations.
Leslie received her master’s in public administration from the University of Pennsylvania. She is the co-editor of the textbook, Age-Friendly Health Systems - A Guide to Using the 4Ms While Caring for Older Adults with Dr. Terry Fulmer, president, The John A. Hartford Foundation, and co-author of numerous publications including: “The Urgent Need for the Age-Friendly Health Systems Movement,” Joint Commission Journal on Quality and Patient Safety; “Advancing Research to Deepen Understanding of the Impact, Implementation and Scaling Up of Age-Friendly Care Using the 4Ms Framework,” BMJ Open Quality; “Evidence for the 4Ms: Interactions and Outcomes across the Care Continuum,” Journal of Aging and Health; and "Age Friendly Health Systems: Pivoting from Breadth to Depth," Inquiry.
