Our Current Funding Strategies

The Foundation’s five funding strategies represent interconnected areas of focus that we view as instrumental to driving systemic, large-scale practice change to improve the health of older adults. The progress we make toward achieving our goals will depend largely on the convergence of these strategies, as no single approach to grantmaking will suffice in addressing the complex array of forces, interests, and issues involved.

 

As we worked through our strategic planning process, we realized that no single grantmaking approach would make the kind of impact needed to drive the large-scale, systemic practice change required to improve care for older Americans. The five funding portfolios we created represent a convergent approach to transform the way health care is delivered to older adults by developing and advancing the best evidence-based models that bring together interdisciplinary teams of health professionals; ensuring that patients and their families are an integral part of the health decision-making process; realigning incentives to reward value, not volume; and supporting policies, regulations, and a health care infrastructure that promote better care.
