Invitation to Join: American Hospital Association's Age-Friendly Health Systems Action Community
The American Hospital Association (AHA) is inviting health systems to join the next Age-Friendly Health Systems Action Community that starts early Fall 2019.
Health systems interested in improving care for older adults at their organization can learn how you can implement the 4Ms Framework for Age-Friendly care, an evidence-based model to support the needs of the aging community in a clinical setting. The 4Ms are: What Matters, Medications, Mentation, and Mobility.
Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI) in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA).
