CHAP Releases Standards and Resources for Age-Friendly Care at Home
Community Health Accreditation Partner (CHAP) has released a new collection of Age-Friendly Care at Home resources and standards.
As part of CHAP's mission to advance the quality of home and community-based care, this release aims to help home health and hospice providers achieve certification and recognition for excellence in quality care for older adults. The resources include learning modules, an admission packet insert, educational podcasts, and more.
One key resource, the Age-Friendly Care at Home Standards of Excellence draft will guide providers towards implementing the Age-Friendly Health Systems 4Ms Framework, which addresses What Matters, Medication, Mentation, and Mobility in the care for older adults. The new standards paired with other educational resources may allow providers to achieve an additional Certification for Age-Friendly Care at Home, starting in April 2023.
