The American Hospital Association is hosting a webinar on August 1, "Creating Age-Friendly Health Systems: An Invite to Join the Action Community."

This webinar will discuss the benefits of joining the Action Community, and the impact of implementing the 4M Framework, in addition to highlighting sites that integrate Age-Friendly care into their hospitals and health systems. Providence Health & Services will share their story of becoming an Age-Friendly health system, and why this movement was necessary for their patient population.

Featured speakers are:

  • Marie Cleary-Fishman, Vice President, Clinical Quality, American Hospital Association
  • Ruth Johanson, Executive Director, Senior Health Program, Providence Health & Services

In partnership with The John A. Hartford Foundation and the Institute for Healthcare Improvement, the American Hospital Association and the Catholic Health Association developed the 4Ms framework for Age-Friendly Care, an evidence-based model to support the needs of the aging community in a clinical setting. The 4Ms are: What Matters, Medications, Mentation, and Mobility.
