Bridging The Home Based Primary Care Gap In Rural Areas

Health Affairs Forefront has published an article, "Bridging The Home-Based Primary Care Gap In Rural Areas."

Access to home-based primary care (HPBC) remains limited in rural areas and addressing this geographical divide would require adapting the design of HBPC to meet the characteristics of rural geographies. An example would be the greater use of nurses and community health workers with direct access to medical providers and augmenting care with virtual support to expand access to specialty care.

Expanding accountable care arrangements in rural areas can further support HBPC models by providing financial stability and flexibility to cover the full costs of operating a rural care practice.

The authors of the article explore opportunities to redesign care models given the unique needs of rural communities and policy and payment reforms to scale HBPC access. Their findings are informed by a literature scan and interviews with leading experts, policymakers, payers and providers.
