The Moving Forward Coalition Nursing Home Quality Coalition (Moving Forward) is holding a Coalition Conversation April 22 on "Why Nursing Home Pharmacists Matter."

During the webinar, Nicole J. Brandt, PharmD, MBA, BCGP, FASCP, will discuss the vital role of the pharmacist in nursing home care, including:

  • How do pharmacists participate in the nursing home team?
  • How can pharmacists help ensure nursing home care meet residents' goals, preferences and priorities?
  • And how can pharmacists help support positive nursing home care transitions in our change health systems landscape?

Dr. Brandt will share her perspective on these issues, as a long-term care pharmacist and national pharmacy thought leader.

She will also facilitate a dialogue with participants about their experiences with nursing home pharmacy – both positive and negative – and how we can include pharmacy and medication management in all our aligned advocacy efforts.

Dr. Brandt will also talk about a new initiative – funded by The John A. Hartford Foundation – to educate and foster pharmacy champions in the Age-Friendly Health Systems 4Ms Framework. The program will form a national advisory council, create a medication optimization learning collaborative across eight regions, and train 2,500 pharmacists and 950 long-term care (LTC) pharmacies, with at least 250 LTC settings earning Age-Friendly Health Systems recognition.
