Nominations Open for The John A. Hartford Foundation 2024 Business Innovation Award

The John A Hartford Foundation BUSINESS INNOVATION AWARD

The Aging and Disability Business Institute at USAging is accepting nominations for The John A. Hartford Foundation 2024 Business Innovation Award. The deadline for nominations is Monday, May 6.

The award recognizes the achievements of community-based organizations (CBOs) that partner and contract with health care entities, such as health systems, health plans, and accountable care organizations, that improve health outcomes and quality of life for older adults, people with disabilities and/or caregivers.

Award recipients are recognized for their bold, transformative initiatives designed to align health and social care and increase the sustainability of their organization. In 2024, applications are especially invited that highlight innovations related to health care contracting and one or more of the following areas related to age-friendly care and the 4Ms (what matters, medication, mentation and mobility):

    • Advance care planning
    • Care coordination
    • Caregiving
    • Dementia care
    • Health equity or health disparities
    • Medication management
    • Person-centered planning
    • Transportation

    2023 award recipients include the Western New York Integrated Care Collaborative (WNYICC), Central Ohio Area Agency on Aging, and Region IV Area Agency on Aging.

    The award - presented by the Aging and Disability Business Institute which is administered by USAging - is given at the USAging Annual Conference and Tradeshow in Tampa, FL, July 8-11, and includes a plaque and monetary award of up to $5,000.
