GIA Webinar: Better Care for Complex Needs: Priorities for the Field

Grantmakers in Aging (GIA) are holding a webinar on November 28th on The Playbook for Better Care which explores the challenges facing adults with complex health and social needs and provides evidence-based or experience-informed ideas about how to address those challenges. The webinar will discuss The Playbook and offer five priorities for this emerging field.

To improve the health outcomes of individuals with the most complex care needs, five foundations - The Commonwealth Fund, The John A. Hartford Foundation, Peterson Center on Healthcare, the Robert Wood Johnson Foundation, and The SCAN Foundation - are working together to accelerate health system transformation. The Playbook, launched in 2016, and developed by the Institute for Healthcare Improvement (IHI), represents one step toward this transformation.

The IHI team has reviewed and created over 150 resources including journal articles, case studies, and practical guidance, and in doing so, has identified priorities for the emerging field that funders may consider as they explore opportunities to improve care for people with complex health and social needs. The IHI speakers are:

  • Kedar Mate, MD, Chief Innovation and Education Officer;
  • Catherine Mather, Senior Project Manager;
  • Marian Bihrle Johnson, MPH, Innovation Director.
