Invitation to Join: Age-Friendly Health Systems Action Community

The Age-Friendly Health Systems initiative is inviting up to 100 teams from a variety of health care settings across the nation to join the Age-Friendly Health Systems Action Community.

The Action Community will help each team's hospital or ambulatory care settings work together to rapidly test and scale-up the specific '4Ms' of the Age-Friendly Health Systems model over a seven-month period to better meet the needs of a growing population of older adults. Becoming an Age-Friendly Health System entails reliably providing a set of specific, evidence-based geriatric best practice interventions to all older adults in your health system.

The instruction and coaching will be setting specific and include transitions in care between settings. The Action Community will begin in September 2018 and conclude in March 2019 (prework will begin in June 2018).

The Age-Friendly Health Systems initiative is a partnership between The John A. Hartford Foundation and the Institute for Healthcare Improvement together with the American Hospital Association and the Catholic Health Association of the United States.
