The National Association of Area Agencies on Aging (n4a)-led Aging and Disability Business Institute has released a crosswalk resource of evidence-based programs delivered by community-based organizations and the Age-Friendly Health Systems initiative's 4Ms - What Matters, Medication, Mentation and Mobility.

An age-friendly approach will measurably improve the quality of care for older adults and optimize value for health systems. Evidence-based programs delivered by community-based organizations have been rigorously tested in controlled settings, proven effective and translated into practical models that are widely available to community-based organizations.
