JAGS Article: "The Age-Friendly Health System Imperative"

The John A. Hartford Foundation (JAHF) and the Institute for Healthcare Improvement (IHI) have jointly authored a paper, "The Age-Friendly Health System Imperative", in The Journal of the American Geriatrics Society.

Significant efforts to move health care from volume to value, along with growth in the population aged 65 and older has created the right moment to envision an age-friendly health system of the future.

JAHF is partnering with IHI on a large-scale initiative in which five early-adopter health systems are prototyping new ways of addressing the complex and interrelated needs of older adults. This work provides great promise for a more-effective, patient-directed, safer health care system.
