2014 Inaugural Hartford Change AGEnts Conference

The Inaugural Hartford Change AGEnts Conference will be in Philadelphia, PA this year from December 3-4, 2014. The conference will gather up 150 Chang AGEnts —fellows, scholars, alumni, mentors and advisors--from the John A. Hartford Foundation’s programs in medicine, nursing, and social work—for a unique, interprofessional leadership conference that can help. This meeting will feature keynote speakers, interactive skill building workshops on a variety of topics, and short-format problem solving sessions—all with an emphasis on helping you apply new knowledge and skills to the change efforts you are already working on.

Would you like to learn how to:

  • Press for care improvements in your home institution or community?
  • Participate more actively in your health system’s redesign efforts?
  • Seek policy changes with local, state and federal policymakers?
  • Shape your research to inform or develop new tools, measures or models of care?
  • Advocate for more geriatrics in the education of practicing clinicians and trainees?

We're gathering up to 150 Change AGEnts—fellows, scholars, alumni, mentors and advisors--from the John A. Hartford Foundation’s programs in medicine, nursing, and social work—for a unique, interprofessional leadership conference that can help.

This meeting will feature keynote speakers, interactive skill building workshops on a variety of topics, and short-format problem solving sessions—all with an emphasis on helping you apply new knowledge and skills to the change efforts you are already working on.

Travel and lodging is included, and participants should complete a brief Statement of Interest by September 24.

*All Hartford Change AGEnts are eligible for this conference. This conference, and the other Change AGEnts opportunities (Policy Institute, Action Awards, Communications Institute, etc.), are NOT mutually exclusive—you may apply for/participate in any or all of them.
